Adventure Dog Camp Application Form

I understand that trips with Adventure Dog Camp (hereinafter 'ADVENTURE DOG CAMP' which term includes related employees, instructors, members, officers, directors and agents) are designed to take myself and other clients to retreat centers in the United States. These locations may include facilities, infrastructure, safety expectations, and other standards that differ from those in my own city, state, country. I understand that Camp may involve physical activity that can be demanding depending on the condition of the traveler or the dog. Every activity has certain inherent risks, and, regardless of the precautions taken, it is impossible to guarantee the safety of the participant. There is inherent risk of personal injury, including permanent disability or death from accident, illness, or the actions of other persons. I understand that other risks associated with Camp include the theft, loss of, or damage to property, including your dog. I understand that some services provided in connection with Camp, including lodging, transportation, food service, local tours, and activities, are obtained from independent suppliers over whom ADVENTURE DOG CAMP may have no control.

I accept that I alone am responsible for the decision to participate in Adventure Dog Camp with my dog. I understand that there are associated risks to my dog or myself with any exercises, including hiking and stand up paddle boarding, that may result in injury. As with any physical activity, I understand that the risk of injury, potentially hazardous or disabling, cannot be fully eliminated. If my dog or I experience pain discomfort, I understand that it is my responsibility to communicate concern and ask for support if needed. I understand that it is my responsibility to obtain a physical/veterinary examination and/or consult with a physician/veterinarian in regards to an outdoor-based program or voluntary exercise program prior to Camp. Without the approval of a physician or veterinarian, I assume all responsibility for my participation in any physical activity or other activity associated with Camp.

Having carefully considered any and all risks involved in taking a trip of this nature, I accept said risks and assume full responsibility for any loss, injury, or damage to me or my dog. This extends to my family and any dependents arising in connection with my participation, or the participation of family members, in Camp. I/we have our own insurance appropriate to our needs in respect to such loss, injury or damage.

I accept that although ADVENTURE DOG CAMP endeavors to choose appropriate independent suppliers to provide services at Camp, ADVENTURE DOG CAMP has no rightful control over their operations. Travel arrangements for Camp are made on the condition that ADVENTURE DOG CAMP is not liable for any negligent or willful act or omission of any independent supplier, or unrelated third party. I acknowledge that the services provided by the independent suppliers are subject to the laws of the place where the services are provided, and any conditions imposed by those suppliers. The liability of the independent suppliers may be limited by their tariffs, conditions of carriage, tickets or vouchers or international conventions or agreements.

I have been honest about my dog’s skills and temperament and understand that by failing to offer accurate information, I am liable for any damages my dog causes to property, people, or other dogs. I agree to manage my dog at all times and respect the space of other participants and their dogs and guests at the lodge. My dog will not be out of my sight at any time. I understand that my dog requires professional e-collar training prior to arrival and failure to do so may result in cancellation without reimbursement. Adventure Dog Camp is not a training program, it is a skill-building retreat for dogs that have already been trained professionally.

I will not sue ADVENTURE DOG CAMP for, and I hereby forever release ADVENTURE DOG CAMP from, all liability for economic loss and physical or mental injury, direct or indirect, arising in connection with Camp. This includes without limitation permanent disability or death; emotional distress; loss of services, financial support, aid consortium or companionship; and any damage to or loss of property.
